Submit a Record Request

CLICK HERE to complete an online medical record request through Healthmark.

    • You will be prompted to enter your patient information.
    • Once you submit the request, you will receive email updates from HealthMark regarding the status, including received, in progress, and completed.
    • Your medical records will be sent to you via CD and will include both the written report and the images.
    • Records take 1-3 business days to process and an additional 3-5 business days to be mailed via FedEx.

For all urgent medical record requests, please complete the form below to pick up your CD at one of our locations. Please allow 24 hours (1 business day) for our team to process your request, prior to arriving at our imaging center to pick up your CD.
